We welcome the opportunity to serve you and look forward to meeting you. As a new patient you will be asked to fill out a complete medical and dental history form. These forms provide vital information which allows us to deliver excellent dental care to you.

Medications and allergies are of a special interest as is a continuing update on your general health. Your information will be kept in your personal file and will not be given out to anyone else.

Typically, your first appointment with Dr. John T. Gotwalt will include a personal interview. This is the time to share personal needs and concerns, address any questions you may have, and discuss the basis for a comfortable, caring doctor-patient relationship. Expect a complete examination of your teeth and soft tissues, diagnostic X-rays and an assessment of your needs for treatment completes your initial exam. A dental prophylaxis is assigned and further needs are addressed after consultation concerning your chosen direction.
